How do I apply for KVP? You must complete and submit an application. To apply for the services offered by the Kentucky Vision Project, you may complete the online application HERE or contact the KVP Office at: 1-800- 320-2406.

How often can I use the program? Due to the high demand, individuals may only use the program one time.

How do you determine if I qualify? Applications are screened by area Salvation Army and Community Action Agencies and Kentucky Home Place to determine eligibility. Eligibility is based on 110% of the current year federal poverty guidelines.

How long does it take to receive an eye exam? After an application is received and approved, the KVP coordinator assigns the applicant to a KVP participating doctor in their area for an eye exam. Most eye exams are scheduled within 6 weeks after approval. However, there are a few counties in the state with a very high volume of applicants, and the waiting time in these counties can be longer.

Do I have to travel a long distance to see the Doctor? No. The KVP has participating doctors all over the state. However, in order to ensure a shorter wait, we do ask each applicant to list at least 2 counties where they would also be willing to travel.

How will I know if I have been approved? Approved applicants will receive a letter from the KVP office with information regarding scheduling their eye exam. If applicants are not approved, they will receive a letter detailing the reason for their denial.

How much does the program cost? The program is free to those who qualify. However, because the Kentucky Vision Project is funded solely by private donations, a donation of $25.00 is requested for those who receive glasses.

Do I need to fill out an application for each person that needs glasses in the household? Yes, each person must fill out their own application.
